Utah uses controversial data for reporting errors

(August 15, 2001, News). On page 1490, paragraph 7 should read as follows: Gulzar H. Shah, programmer and analyst for Utah's Department of Health and one of the authors of the study, said his state recently proposed administrative rules that would mandate the reporting of certain critical adverse events. The rules would also require hospitals to have programs to track and reduce the frequency of adverse drug events. On page 1494, the third-to-last paragraph should include the Utah Hospital Association as a partner.
