Go to the [ COST ] chart to find the category and description that best matches your institution. This will determine your appropriate tier and pricing.

Phone the ASHP Customer Service Center toll-free at 866-279-4536 to place your order. Representatives are available 7 am to 7 pm ET Monday through Friday.

After placing your order, the customer service representative will give you your ID number and password. Your administrator will then be able to go to www.ajhp.org/subscriptions. Clicking on Activate Your Institutional Subscription will give them access to set up your range of IP addresses.

If you are ordering through your serial vendor, once ASHP has received payment, you will receive a letter with instructions for obtaining access to your Institutional subscription. You will be asked to provide the following information necessary to activate your subscription to Am J Health-Syst Pharm Online:

• Institution’s IP Address(es)
  [CRITICAL INFORMATION ABOUT IP ADDRESSES -- READ THIS]
• Contact name, title, phone, fax and email
• Customer number (included with the letter).

We will send you the address of the web form after we verify payment. Your institution will not have access to the full Am J Health-Syst Pharm Online until you complete this second step.

If you are having access problems...

Access to Am J Health-Syst Pharm Online is now restricted to print subscribers of the journal. A letter has been sent out instructing subscribers on how to sign on to the online journal. If you are having difficulties in accessing the material, please read the following instructions:

If you are subscribing to Am J Health-Syst Pharm directly through ASHP (not using a book agent), please give us a call at 1-866-279-0681 and we will try to assist you. Make sure that the subscription ID number we provided you with is available.

If you are subscribing to Am J Health-Syst Pharm through a book agent and the issues are being shipped directly to your address, please give us a call at 1-866-279-0681 and we will try to assist you. Make sure that the subscriber ID number we provided you with is available.

If you are subscribing to Am J Health-Syst Pharm through a book agent and the issues are being sent to them at their collection place, please call your book agent and ask them to provide us with documentation that identifies you as a subscriber. Without documentation, you will not be identified as a paid subscriber and we will not be able to help you from our office.
